Data Use Agreement

Health Workforce Database data are available for research, analysis and aggregate statistical reporting and are password-protected. Data, analysis or information should not be used to identify individual survey respondents directly or by inference.

CHI requires that individuals or organizations downloading the Health Workforce Public Use File databases fill out this data use form. This agreement is required to ensure the privacy of survey respondents.

The Colorado Health Institute (CHI) of Denver, Colorado and (Principal Investigator) agree to the following conditions regarding the use of CHI's Health Workforce dataset named:
2008 Colorado Rural Dentist Survey
2006 Colorado Certified Nurse Aide Survey
2006 Colorado Dentist Hygienist Workforce Survey
2007 Licensed Practical Nurse Survey
2006 Colorado Dentist Workforce Survey
2005 Colorado Physicians Workforce Survey
2005 Colorado Pharmacist Workforce Survey
2005 Colorado Registered Nurse (RN) Workforce Survey

The survey data are to be considered confidential.
Published reports of survey results or analyses of survey data must be in aggregate format only, with no values provided for cell sizes smaller than five and no individual-level information released.
Neither record-level survey data nor any portion of the dataset thereof will be distributed or released to another party.
The survey data shall only be used for the purposes of the study outlined in Attachment A.
CHI will be cited as the data source, using the following disclaimer statement in any public release of data or analysis, or in any publication or research based upon these data:

“Source: These health workforce data were supplied by the Colorado Health Institute, which specifically disclaims responsibility for analyses, interpretations or conclusions it has not provided."

CHI and the Principal Investigator named in this agreement are bound to the terms and conditions set forth herein.
