Registration fees for the symposium are:

$20      On or before April 26, 2007
$40      After April 26, 2007

• Late registration: If you were unable to meet the April 26 registration deadline, you may still register on-site at the Denver Marriott West. Bring a check, money order or cash to pay the $40 registration fee on-site.

All cancellations must be in writing and faxed to 303.831.4247 or emailed to helmk@coloradohealthinstitute.org no later than April 30, 2007 in order to receive a refund. Participants are responsible for canceling their hotel accommodations.
